Every appointment fee is listed below, along with how Medicare and DVA can reduce what you pay. Fees are the same at all four clinics and for telehealth.

Appointments

What each appointment costs

Most people see Michelle on a GP Care Plan, which attracts a Medicare rebate. If you do not have a care plan you are still very welcome to book privately.

With a GP Care Plan

Referred by your GP under a chronic condition management plan. A Medicare rebate applies.

Initial diabetes consultation
45 minutes. Full assessment, education and a plan you can act on.

Private appointments

No referral and no care plan needed. Book whenever you are ready.

Rebates

Ways to reduce what you pay

Medicare

GP Care Plan

If your GP puts you on a chronic condition management plan, Medicare pays a rebate of $63.40 towards your initial consultation with Michelle, and a rebate also applies to your follow up appointments. Medicare currently allows up to five allied health services each calendar year, shared across every allied health provider you see, and your GP decides how those are allocated.

DVA

Veterans

DVA clients are accepted. Bring your DVA card and a valid referral from your GP to your first appointment, and Michelle will confirm how your appointments are covered before you are charged anything.

Private health

Extras cover

Michelle is an ADEA Credentialled Diabetes Educator, and many extras policies include diabetes education. Rebates depend on your fund and your level of cover, so check with your fund before your appointment.

Good to know

Payment and cancellations

When do I pay?
Payment is made on the day of your appointment. Telehealth reviews are the exception and are paid before the appointment starts.
What happens if I need to cancel?
Please give at least 24 hours notice so the appointment can be offered to someone else. Cancellations inside 24 hours may be charged.
Do I need a referral to see Michelle?
No. You can book a private appointment at any time without a referral. A referral from your GP is only needed if you want to claim the Medicare rebate under a care plan.
Do fees change from clinic to clinic?
No. The same fees apply at Croydon, Lilydale Medical Centre, Main Street Medical Lilydale, Rowville and for telehealth.
Is insulin pump support charged differently?
No. Insulin pump education, pump starts and pump reviews are booked as standard consultations at the fees listed above. Pump work often suits the extended review appointment, which allows more time. More about insulin pump support.
What should I bring to my appointment?
Please bring your most recent blood test results if you have them, including HbA1c and fasting glucose, plus lipids and eGFR if possible. Also bring your glucose meter, sensor or pump, your current medicines list, and your Medicare or DVA card.
How do I claim the Medicare rebate?
You pay the full fee on the day and the rebate is claimed back to your nominated bank account. Bring your Medicare card and your care plan referral so this can be processed for you.
